Things You Should Take With You
- Valid Drivers License (Be sure that your DL won't expire during your incarceration, your Unit Team will be able to assist you on renewal)
- Social Security Card
- Certified Birth Certificate
- Original Copy of DD-214 (If possible, get a certified copy)
- Certified High School Transcript (Must be a certified copy)
- Medications (Include Prescriptions)
- Medical Records (Any records that the BOP will need to continue your care while incarcerated)
- Vaccine Records/Card
- Eye Glasses with supporting Prescription
- Contact Lenses and Case (No Solution, Purchase in Commissary)*
- Wedding Band (Can Not Be Valued Greater Than $100.00, No Stones, Diamonds, etc)*
- Contact Names, Phone Numbers, Addresses and Email Addresses
- Lawyers Name, Address, Phone Number and Email Address
- All Legal Documents You Will Need For Your Case
- Power of Attorney Signed and Notarized
- Living Will - Medical Directive Signed and Notarized
- C-Pap Machine. *Recommend you contact the Designated Facility directly and discuss with the R&D Department about what machines are permitted. Must have Prescription.
Things you Should Attend To Prior To Surrendering
- Last Will and Testament - Notarized (Store in a secure location)
- Bank Information (Account Management while Incarcerated, POA's, Family Access, etc.)
- Social Security Administration (You are not permitted to receive SSI while incarcerated. Be sure to stop all payments in advance)
- Veterans Administration (Check with the VA to determine any benefits you are allowed to receive while incarcerated)
- Retirement Administrators (Continuing Payments, Payment Mailing Addresses)
- Social Media Information
- Cell Phone Information (Closing the account, freezing the number, continuing service while incarcerated)
- United States Probation Office Information
